       答. 身心障礙者專用停車位識別證，如註記資料有模糊無法辨識之情形，可檢具申請表，身心障礙手冊正反面影本，申請人行車執照及駕照影本，及原停車證至本局服務站辦理換發，或以掛號郵寄至本局辦理換發。
