       答. 1、設籍本市，領有身心障礙手冊之身心障礙者本人，或需承載身心障礙者之同戶籍家屬。
             2、身心障礙者（領有身心障礙手冊）本人或身心障礙者同一戶籍內家屬，1戶以申請1張為限。同址分戶不得申請。
